Applications, Procedures & Forms

Application Procedure

  1. Family tour of our school
  2. Completion of our enrolment forms
  3. Photocopy submission of:
    I. Child’s current immunization record
    II. Child’s birth certificate and/or passport, citizenship/visa documents
  4. Disclosure of child’s medical conditions (if any) and current medications
  5. Release to obtain child’s Ontario Student Record (OSR)
  6. 10 postdated cheques for registration and tuition fees
